Position: Personal Care Management (PCM) Skills Trainer

$250 HIRING BONUS!

Full-time: Provides guidance and training to individuals with disabilities seeking services through the Medicaid Personal Care Attendant (PCA) Program. Responsibilities include conducting home assessments, offering training on PCA program management, and assisting consumers with program eligibility. The role involves troubleshooting program issues, ensuring timely documentation, and maintaining consumer files. The trainer works closely with Fiscal Intermediaries and participates in departmental meetings. Travel throughout Worcester County is required.
$19.00 per hour

Position: PRN & LPN Clinical Evaluator

 Per-diem: Conducts assessments of consumers’ physical and cognitive functional status in their homes to support the PCA and MRC Home Care Assistance Programs. The role involves making recommendations to enhance consumer safety and independence, collaborating with occupational therapists, and maintaining necessary documentation. Candidates should have an active RN or LPN license in Massachusetts, experience in community healthcare, and familiarity with disability services. The position requires travel within the service area. $100 for Initial evaluations & $80 for annual re-evaluations, plus mileage.

Position: IL Long Term Support & Services Coordinator, Spanish Speaking

Full-time: Responsible for assisting individuals aged 21–64 who are dually eligible for Medicaid and Medicare and enrolled in One Care Organizations. The coordinator helps assess, arrange, and monitor long-term support services (LTSS), advocates for consumers’ needs, and ensures delivery of person-centered, community-based care. This role requires strong communication, planning, and documentation skills, cultural competency, and experience with disability services. Spanish language skills are preferred. Travel and home visits throughout Worcester County is required. $20.10 per hour

Position: Deaf and Hard of Hearing Independent Living Services (DHILS)
Deaf Skills Trainer

$250 HIRING BONUS!

30 Hours per Week; With Benefits: Provides Independent Living skills training and peer counseling to Deaf consumers. The role involves conducting intake assessments, developing personalized Independent Living plans, advocating for Deaf services, and offering emergency intervention when needed. The position requires fluency in American Sign Language (ASL) and experience working with the Deaf community. The trainer will also attend workshops, maintain accurate records, and may assist with college interns. Travel within the service area is required – Valid driver’s license and a vehicle are required.
$19.00 per hour
