Options Counseling

Many times, the difference between maintaining our independence within the setting of our choosing, or entering into a nursing home or institutional life style may be the lack of knowledge as to what resources are available to us in order to maintain our independence. In partnerships with the Aging & Disability Resource Consortium (ADRC), Options Counselors at Center for Living & Working strive to provide consumer-directed support to consumers, family members, caregivers and/or significant others in hospitals, nursing facilities, rehabilitation facilities, and community settings who are planning for long term services and supports.

Through the  no wrong door model, our Options Counselors are here to actively listen to your concerns, and help to connect you to resources and supports within your community to help maintain your independence.
